Free Individual Care Plans— Your geriatric care manager first meets with you to determine what the status of the patient is and then tailors a plan to meet the unique needs of the person and the situation. There is no charge for this meeting as we believe it is only fair that you have an opportunity to get to know us and the way we work first.

Geriatric Care Management Services

• Regular Assessments — We provide regular consultation and assessments of our patients. It gives you an opportunity to get an up-to-date, independent opinion on the state of health of your loved one.
• On Site Care Management — If you live out of town or are especially busy with other aspects of your life, we will do what you would do if you were on the scene. This can involve everything from planning the care to keeping the household running smoothly to assisting with any transitions to other facilities as required. We're here to help you work through any of the myriad of problems that can arise...even if you can't be here yourself.
• Assistance with Alternative Living Options — We can help the patient make as smooth and happy a transition to other living facilities as possible. We also frequently have aides who will be with our patients while they are living in assisted living communities.
• Consolidation of Community Resources — We'll help coordinate the healthcare of your family member including making sure that doctors, physical and occupational therapists, and pharmacists are all on the same page.

Registered Nurse Supervision — Periodic home visits with home health care aide supervision.

Our Coverage Area — We serve people who live anywhere in the Greater Boston area including the outer suburbs. Please call us for specifics.

Fees — For information about our fee structure, please call us and we'll be glad to give you the details. Geriatric Resource Centre now accepts Long Term Care insurance so be sure to ask us whether you qualify.
